Outreach and Engagement Worker – GAPPS (Getting Appropriate Personal and Professional Support)

Full-Time Permanent – 1 Vacancy

70 Hours Biweekly

Rate: $29.50 - $31.35/hour (based on experience)

These positions are within OPSEU Local 720 Bargaining Unit

Canadian Mental Health Association – Thunder Bay Branch is a non-profit service provider working to ensure mental health services are available for all. CMHA Thunder Bay is committed to providing quality services for all of Northwestern Ontario residents where all staff strive to protect and promote mental health for all through healthy lifestyle and exemplary recovery focused approach improving upon each individual’s well-being.

What is the job?

The Outreach and Engagement Worker’s priority is to facilitate outreach and engagement activities to the following target population: Individuals with a serious mental illness and/or addiction, have a history of not engaging with the “traditional mental health/addiction services, have a need for episodic urgent service however have had frequent re-entry and/or inappropriate access to access points within the health care system (i.e. emergency room), have considerable difficulty with engaging with formal health care providers including having challenges committing to treatment/support plans, and who may be involved with the criminal justice system.

The Outreach and Engagement Worker(s) will work in partnership with other community partners, to facilitate access to services along the service continuum.

Service is delivered in non-traditional settings and may include evenings and weekends.

What will you do day to day?

Demonstrating ability to effectively engage with the identified target population
Developing and sustaining community partnerships within and external to the mental health and addictions service continuum
Performing comprehensive bio-psychosocial assessments and identifying risks
Assisting the target group in navigating health, mental health/addiction and social services, including housing
Working with the client and in consultation with the team facilitating timely access to clinical services associated with this project and with the formal and informal care/service/support system
Maintaining ongoing connection with the target group
Demonstrating active involvement within the team planning process in the development of treatment and recovery plans including activities, expected outcomes and time frames that produce expected results as well as a mechanism for reporting consistent progress towards achieving identified goals
Ensuring all documentation, reporting and file management functions are addressed
Ensuring supplies are purchased and approved through proper handling of agency funds
Ensuring accurate records, receipts, mileage statements, and expense claims are maintained with appropriate forms
Ensuring collection of statistics, reports, incident reports, on-call reports, weekly time sheets, and performance appraisals are submitted to the Program Supervisor/Director
Adhering to and promoting CMHA’s Code of Ethics Policy
Complying with CMHA’s Health and Safety Policies
Completing other duties that may be assigned

What do you bring to the job?

Degree or Diploma in Social Sciences
Minimum 2 years direct experience working with individuals with serious mental illness and addictions issues who are not effectively engaged with formal services (homeless)
Demonstrated knowledge and skills in bio-psychosocial assessment
Ability to work collaboratively within a multi-site/inter-agency team
Knowledge of health, mental health, addictions, and social services
Ability to work independently with minimal support and supervision
Functional computer skills
Relationship building
File updating and maintenance
Problem solving, organizational, listening, and verbal skills
Ability to work in a team environment
Police Vulnerable Sector Check required
Ability to travel and use personal vehicle
Valid Class “G” Driver’s license with 2 million liability insurance
Legally eligible to work in Canada
Ability to speak French is considered an asset
Availability to work variable hours: days, evenings and weekends
