Position Summary
The Director, North York Toronto Health Partners Ontario Health Team (NYTHP OHT) and NYG Community Integration , is responsible for operationalizing and implementing the OHT strategy, by providing leadership, coordination and support to key aspects of the development, implementation and growth of the OHT. The director will lead the development and implementation of strategic priorities, be responsible for the operational and resource plan and annual budget for the North York Toronto Health Partners OHT. The Director will represent and advocate/advise on NYTHP’s position directly through participation in consultations, submissions and meetings with regional and provincial decision-making bodies including, but not limited to, Ontario Health, Ministry of Health, and other Ontario Health Teams. This position provides strategic oversight of NYG-led community integration programs, driving partner alignment, performance, and sustainable scale to advance primary care attachment, reduce ED utilization, and enable seamless hospital–community care. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for community, patients/clients, caregivers, staff, physicians, volunteers and families.
On a practical level, you will
- Lead the development and implementation of the NYTHP OHT strategic plan to improve population health for the community
- Proactively position NYTHP OHT to excel and be a leader among OHTs by monitoring the provincial OHT landscape and best practices in the development of integrated health systems
- Identify, escalate and manage risks, including ensuring ongoing risk tracking and management and providing direction on risk mitigation strategies
- Develop effective and collaborative working relationships with the members of the Stewardship Council, the 22 NYTHP OHT core partners, NYTHP OHT Primary Care Network, NYTHP OHT Patient and Caregiver Council, NYTHP OHT affiliate partners, Ontario Health, the Ministry of Health and other stakeholders to promote alignment and coordination of priority work streams, information exchange, and effective operations.
- Develop and implement a robust and inclusive community engagement strategy in collaboration identified partners.
- Identify key partnership and strategic opportunities and work with identified stakeholders to develop business cases to advance NYTHP OHT priorities.
- Lead, supervise, coach and evaluate the OHT Backbone Team.
- Develop work plans, project plans and ensure timely and effective completion of all required deliverables.
- Develop and implement a communication strategy in partnership with the core and affiliate partners that supports the work of the NYTHP OHT and provide regular updates to key stakeholders.
- Develop budgets, operating plans and reports on NYTHP OHT activities which are required by the Ministry of Health and Ontario Health.
- Develop and implement structures, processes, and tools to enable integrated delivery of results.
- Provide support to the Stewardship Council, including agenda planning, report generation and ensuring that secretarial support is available for meetings.
- Supervise, manage, coach a team of staff supporting NYTHP work and provide direction and mentorship to a broader backbone team of professionals.
- Collaborate with internal and external stake holders; Advise on opportunities for integration and innovation across the hospital and community providers.
- Facilities collaboration and capacity-building among the NYTHP OHT Patient and Caregiver Health Council and the NYGH Patient and Family Advisors.
- Ongoing monitoring, reporting of budget with Ontario Health Team Partners and Finance Department
- Provide strategic and operational oversight of NYG-led community integration programs (e.g., NYCAST/NLOT, SCOPE/LTC+, Palliative), ensuring alignment to 24/7 care, primary care attachment, and seamless hospital–community pathways.
- Lead collaboration with NYTHP partners; oversee governance (e.g., MOUs, collaboratives) and monitor program performance (e.g., referrals, LTC coverage, access/utilization) to drive impact on ED avoidance, attachment, and transitions of care.
- Advance program planning, funding alignment, and scalability; support IPCT/PCAT priorities, assess sustainability, and enable disciplined growth of high-impact models in line with system demand and Ontario Health direction.
Qualifications
- Undergraduate degree and post-graduate training in health administration, business, public policy and or a related discipline, required.
- A minimum of 5 years progressive leadership experience in a healthcare or related environment working with dynamic and diverse teams and managing complex, large-scale projects.
- Deep understanding of the Collective Impact model.
- Demonstrated skills in project management and quality improvement.
- Proven ability to develop coalitions, foster a collaborative team approach to decision- making, work across sectors and across organizations to achieve collective outcomes and sustain relationships with diverse, local health care and social service leaders.
- Experience managing and leading interdisciplinary teams, collaborating and building partnerships with leaders and experts from a broad range of stakeholder groups, and providing leadership in strategic planning, operational planning and performance management and reporting, required.
- Provide leadership to advance health system integration initiatives across NYTHP. This would include developing pilot programs, advocating for funding, evaluating initiatives and helping to spread and scale leading projects.
- Work to reduce and eliminate barriers to health system integration across partners in North York. This includes developing innovative pathways, digital supports and staffing that help bridge identified gaps so partners can quickly execute on ideas.
- Take a mindset that anything is possible with the right drivers and levers in place. This would include challenging ways of operating that keep organizations in their silos and championing system wide solutions over individual organizational desires.
- Extensive Knowledge and Experience with the Ontario Health Care System and Health Policies. Direct Community Integration experience preferred.
- Ability to build authentic stakeholder relationships and facilitate complex, multi-sector partnerships and lead community engagement strategies.
- Exceptional writing, presentation and interpersonal communications skills.
- Strong program management experience, including demonstrated ability to efficiently plan, organize, and manage multiple complex projects simultaneously.
- Knowledge and practical experience in the areas of health equity and population health planning
- Experience with government and directly supporting Boards and or similar governance bodies, an asset
- Evidence of innovative leadership skills and proven experience in system transformation and change management.
- Skills in negotiation, conflict resolution and consensus building with a participative/consultative style that brings consensus.
- Strong understanding and analysis of population health methodologies, health system planning, health system utilization data and integrated care.
- Knowledge of health equity, the social determinants of health and local health issues and the North York context.
- Knowledge and understanding of the relevant legislation pertaining to OHTs and related stakeholders.
- Experience with developing and managing budgets and reporting of financial and other key metrics.
- Excellent interpersonal skills with the ability to foster positive and respectful internal and external relationships/partnerships, including relating effectively with front-line staff and all levels of management, physicians, board members, clients/family members and the public.
- Excellent verbal and written communication skills (e.g. group facilitation, public speaking, written reports, and verbal presentations will be frequent activities).
- A clear, demonstrated commitment to ethics, equity, diversity, inclusion, anti-racism and anti-oppression.
- Strong working knowledge of computer applications, notably Microsoft Office.
- Bilingualism in French/English, an asset.
