Job Overview

We are seeking a dedicated and detail-oriented Clinical Pharmacist to join our healthcare team. The ideal candidate will provide expert medication management and patient care within a hospital setting, ensuring safe and effective use of pharmaceuticals. This role offers an opportunity to work in a dynamic environment, collaborating with healthcare professionals to improve patient outcomes. The Clinical Pharmacist will be responsible for reviewing prescriptions, counseling patients, and supporting clinical decision-making processes. Candidates with hospital experience, especially in acute care or pediatrics, are highly preferred.

Responsibilities

Review and interpret physician prescriptions to ensure accuracy and appropriateness of medication therapy
Provide expert consultation on medication selection, dosage, interactions, and side effects
Counsel patients on proper medication usage, potential side effects, and safety precautions
Collaborate with healthcare teams to develop and implement individualized patient care plans
Administer medications as prescribed in accordance with hospital policies and medical terminology standards
Monitor patient responses to medications and adjust therapy as needed for optimal outcomes
Maintain accurate documentation of medication administration and patient interactions
Stay current with new pharmaceutical developments, guidelines, and best practices in patient care
Experience in sterile and non-sterile compounding is preferred
Experience in homecare/home infusion processes is preferred

Requirements

Valid license as a licensed Pharmacist with relevant state certification
Hospital experience is preferred; acute care experience is highly desirable
Knowledge of pediatrics and specialized medication protocols for children is a plus
Strong understanding of medical terminology and medication administration procedures
Excellent communication skills for effective patient counseling and interdisciplinary collaboration
Ability to work in fast-paced environments while maintaining attention to detail
Commitment to providing high-quality patient care with a focus on safety and efficacy

Pay: $43.33-$64.91 per hour
