OPPORTUNITY
We are hiring a Community Engagement Worker (CEW) to join our team. These full-time positions are an opportunity to put your knowledge and technical skills to work on a variety of member focused related tasks. Under the direction of the Community Service Supervisor, the Community Engagement Worker provides supports to individuals with disabilities through the creation, implementation and evaluation of member directed services. This position coordinates, and facilitates individualized, group and virtual services based on specific member service contracts. Services are member directed and designed to promote personal welfare, independence, education, self-determination, community participation, recreation and social skills.

THE ROLE:
-
Ability to develop and demonstrate an empathic working relationship with the members and their caregivers. Able to understand, communicate and demonstrate member needs, wishes and progress.
- Curriculum development for individualized (1:1), group and virtual activities. Management of current service resources and virtual curriculum library.

Implementation, and evaluation of service (curriculum / activity) based on member’s Individual Service Agreement(s).

Coordinate service (curriculum) offerings to members across multiple VITA locations.

Based on member service contracts, research, community activities and develop strategies for member access. This includes identifying all required resources for member’s full participation i.e., transportation, staffing support, use of technology and community fees.

Identify members’ need for specialized services; and provide the Supervisor recommendations of required resources, referrals and or additional external supports needed. Access required supports and complete all required documentation.

Track member participation, service evaluation and or satisfaction surveys to identify trends to modify and or improve future service offering.

Maintain accurate and up to date records of member supports through VITA’s member data base and the completion of all service documentation.

Ongoing evaluation of member needs and service contract goals, advocating, communicating, and adjusting service supports required to meet member objectives.

Responsible for ongoing service assessment, identifying member challenges and successes and coordinating various service meetings with members, family, primary care providers and or community partners.

When required, administration and documentation of member prescribed medication as per VITA’s Medication Policies and Procedures.

Follow VITA and program documentation requirements (i.e., communication book, daily logs, seizure reports, progress reports, P-Card submission etc.).

May be required to assists members with personal care and hygiene, as necessary. This can include encouraging independence, assistance with personal hygiene, feeding, dressing and position change, as well as care, maintenance, and use of any equipment required for the support members.
WHAT YOU BRING:

D.S.W, SSW, CYW or University Degree in a related field or other relevant post-secondary education.

Minimum 2 years’ experience working with individuals with disabilities and/or dual diagnoses.

Experience with activity development, individual and group facilitation.

Experience with service coordination, across multiple program sites and platforms.

Must demonstrate, have strong working knowledge of technology & computer skills, including, Microsoft Office Suite, virtual platforms (I.e., Teams / Zoom). Social media and other general use of Apps

Valid driver’s license required
-
Valid and on-going current training certificates in Abuse training, Back Care, First Aid, Fire Safety, Safe Management, Pharmacology and WHMIS.
