Full job description

Job Title: Primary Health Care Nurse Practitioner
Job Type: Full-time, Schedule: Flexible (may include evenings, weekends, and on call)
Salary: $119,900 – $142,700 per year (prorated based on hours), inclusive of an on-call stipend.

Gateway Community Health Centre, located in rural Ontario, provides comprehensive primary care, dental services, and allied health support. Our work focuses on health promotion and illness prevention, delivered through a collaborative, inter-professional team.

We serve individuals and families at every stage of life, with a particular emphasis on those at higher risk or facing barriers to accessing care. The Primary Health Care Nurse Practitioner will conduct comprehensive health assessments and physical examinations across the lifespan, using evidence-informed clinical reasoning to formulate diagnoses. The role involves collaborating with clients and interdisciplinary care teams to develop and coordinate individualized care plans, as well as identifying trends in client health needs to inform program planning and advocacy efforts. The incumbent will also be responsible for maintaining clear, accurate, and timely documentation, including electronic medical records, reports, and requisitions.

Qualifications

Current registration with the College of Nurses of Ontario as a PHCNP
Proof of Liability Insurance
Minimum of five (5) years of clinical experience in a healthcare role, ideally in primary care or community health, demonstrating readiness for advanced practice as a Nurse Practitioner.
Proficiency in Microsoft Office and electronic medical record systems
