Job Summary
Under the direct supervision of the Operations and Wellness Director, the Registered Nurse (RN) is an integral member of Lu’ma Medical Centre Society’s interdisciplinary primary care team, providing comprehensive, culturally safe, trauma-informed, and patient-centered nursing services to Indigenous and non-Indigenous clients across the lifespan. Working within an innovative team-based primary care environment, the RN supports Lu’ma’s mission of improving access to holistic health and wellness services for urban Indigenous peoples and their families.
The RN delivers evidence-informed nursing care through health assessment, chronic disease management, health promotion, disease prevention, care coordination, patient education, and clinical interventions. The position emphasizes building respectful and trusting relationships with clients by incorporating cultural humility, anti-racism, and trauma- and violence-informed approaches to care. The RN works collaboratively with physicians, nurse practitioners, social workers, Elders, traditional wellness providers, and community partners to address the physical, emotional, mental, spiritual, and social determinants of health.
The Registered Nurse actively contributes to creating a welcoming, inclusive, and culturally safe environment that honours Indigenous ways of knowing, being, and healing. Nursing services are provided in accordance with the standards of practice established by the British Columbia College of Nurses and Midwives (BCCNM), applicable legislation, and organizational policies and procedures.
Responsibilities
· Conduct comprehensive and holistic nursing assessments, including health histories, physical assessments, medication reviews, risk assessments, and wellness evaluations.
· Monitor, assess, document, and evaluate client health status, symptoms, interventions, and outcomes using evidence-informed nursing practices.
· Provide direct nursing care, treatments, medications, immunizations, injections, and clinical procedures within the RN scope of practice and established protocols.
· Order and/or collect diagnostic tests and specimens, where appropriate and within scope of practice, and accurately document results and clinical findings.
· Support chronic disease prevention and management, including diabetes, cardiovascular disease, respiratory conditions, mental health concerns, and other complex health needs.
· Promote health literacy, self-management, preventative care, and healthy lifestyle choices through patient education and coaching.
· Participate in care planning, case management, and follow-up activities to ensure continuity of care and positive health outcomes.
· Demonstrate cultural humility and provide care that respects and incorporates clients’ cultural values, beliefs, traditions, and lived experiences.
· Apply trauma- and violence-informed practices that recognize the impacts of colonialism, systemic racism, intergenerational trauma, and social inequities on health and wellness.
· Foster a culturally safe, anti-racist, and inclusive care environment for clients, families, and colleagues.
· Support the integration of Indigenous perspectives, traditional wellness practices, and Two-Eyed Seeing approaches alongside Western clinical care.
· Advocate for equitable access to health services and resources that address the social determinants of health.
· Collaborate with clients, families, caregivers, Elders, traditional wellness providers, and interdisciplinary team members to develop and implement individualized care plans.
· Coordinate referrals and facilitate access to internal and external health, social, and community services.
· Participate in interdisciplinary case conferences, team meetings, quality improvement initiatives, and organizational planning activities.
· Communicate effectively with healthcare providers and community partners to support integrated, coordinated care.
· Maintain accurate, timely, and confidential clinical documentation in electronic medical records and other required systems.
· Maintain professional competence through continuing education, training, and engagement with current nursing evidence and best practices.
· Participate in organizational initiatives that support quality improvement, cultural safety, anti-racism, and patient-centered care.
· Adhere to all professional, ethical, legal, and organizational standards and requirements.
Perform other related duties as assigned.
Qualifications
· Bachelor of Science in Nursing (BSN) from an accredited nursing program.
· Current practicing registration in good standing with the British Columbia College of Nurses and Midwives (BCCNM).
· Current CPR/BLS certification.
· BCCDC Immunization Certification.
· Minimum three (3) years of recent nursing experience in primary care, community health, public health, Indigenous health, chronic disease management, or a related healthcare setting.
· Experience working with Indigenous communities and providing culturally safe care, anti-Indigenous racism, trauma-informed care, and harm reduction.
· Experience with electronic medical records and clinical documentation systems.
· An equivalent combination of education and experience may be considered.
· Understanding of, respect for, and commitment to Indigenous cultural practices, traditions, and healing approaches.
· Knowledge of social determinants of health and available community resources and support systems.
· Understanding of Indigenous cultural safety, trauma-informed practice, and harm reduction approaches.
· Knowledge of primary care nursing, population health, health promotion, disease prevention, chronic disease management, and care coordination.
· Strong clinical assessment, critical thinking, and decision-making skills.
· Excellent communication, interpersonal, and relationship-building skills.

· This position may require sitting, standing, and walking for extended periods throughout the workday. Frequent use of computers and electronic medical records. Ability to perform clinical assessments, administer treatments and immunizations, and assist clients with mobility as required.
