Reports to: Health & Wellness Manager

Location: 291 Main St., Brighton

Status: Permanent, Part-Time

Salary/Wage: $30.00/hour

Hours of Work: 30 hours per week + every other weekend

Reporting to the Health & Wellness Manager, the Registered Staff (RPN) provides professional nursing care to residents in a retirement home setting in accordance with established policies, procedures, and regulatory requirements. The RPN collaborates with other healthcare professionals, caregivers, and support staff to promote residents' physical, emotional, and psychological well-being. This role is central to delivering safe, compassionate, and person-centered care tailored to the unique needs of older adults.

Duties and Responsibilities:

1. Resident Care

1.1. Deliver direct nursing care to residents, including medication administration, treatments, wound care, and monitoring of chronic conditions.

1.2. Conduct routine assessments and document changes in residents' health status.

1.3. Develop and implement individualized care plans in collaboration with the healthcare team.

1.4. Respond promptly to emergency situations and initiate appropriate interventions.

1.5. Administer medications in accordance with physician orders and applicable regulations.

1.6. Monitor residents for side effects and interactions, and report concerns to attending physicians.

1.7. Maintain secure medication storage and inventory control.

1.8. Advocate for residents’ rights and preferences in care decisions.

1.9. Provide emotional support to residents and families dealing with illness, loss, or transitions.

1.10. Promote resident independence and dignity.

1.11. Oversee medication administration program.

1.12. Oversee admission and discharge of residents; make appropriate discharge plans.

1.13. Supervise resident person hygiene and bathing, per schedule, and assist when necessary.

1.14. Collaborate with volunteer and staff in the development and implementation of activity programs and outside services.

2. Clinical Documentation and Reporting

2.1. Accurately chart care provided and observations in resident health records.

2.2. Report any changes in resident condition to the appropriate personnel and family members as required.

2.3. Maintain compliance with documentation standards, including electronic health records (EHR) systems.

2.4. Ensure all medical records and personal health information is maintained per PHIPA.

2.5. Organize and review resident medications.

2.6. Ensure resident care plan is established and reviewed at least every six (6) months.

2.7. Complete and follow-up on incident reports when necessary.

3. Leadership

3.1. Provide direction and supervision to all care staff.

3.2. Monitor staff performance and complete performance evaluations during probationary period and on an annual basis.

3.3. Responsible for leadership coaching and mentoring to all Unregulated Care Providers.

3.4. Participates in staff development, quality assurance, safety, environment and infection control programs.

3.5. Ensure staff involvement in fire and safety procedures and communication to all staff of inspection reports and follow-up actions.

3.6. Ensures nursing documentation is complete as per College of Nurses’ Nursing Documentation guidelines.

3.7. Ensures there are sufficient staff on duty to provide the services which are offered and/or required.

4. Risk Management and General Safety

4.1. Partners with community team to ensure community is in compliance with Occupational Health and Safety requirements and promotion of Risk Management programs and/or policies, adherence to safety rules and regulations.

4.2. Practices safety procedures at all times including personal protective equipment (PPE), fire safety procedures (extinguishers), safety data sheets (SDS), and lockout tagout procedures.

4.3. Reports all accidents and/or incidents immediately.

4.4. Reports all unsafe and hazardous conditions and/or equipment immediately.

4.5. Complies with all infection control techniques, placement of biohazard containers and removal techniques, procedures and policies.

4.6. Participates in fire safety and mock disaster procedures.

5. Other

5.1. Works in a safe manner and ensures department operations are carried out in a safe manner in accordance with the Occupational Health and Safety Act, associated regulations, Legacy Living policies, procedures, and other associated legislation.

5.2. Other related duties, as assigned.

The successful candidate will possess the following:

Education

Degree or diploma in gerontology, rehabilitation, community/complex/long-term care.

License, Registration, Training and Clearances

Police Record Check (per RHA), including Vulnerable Sector Check (VSC)
First Aid and CPR certification.
Valid and unrestricted Class “G” Driver’s License with access to reliable transportation.
Immunization and Tuberculosis (TB) testing, per Brighton Retirement Living’s policy.
Valid license with the College of Nurses of Ontario (CNO).
Must maintain good standing with the regulatory college, including regular continuing education and renewal.

Experience

Three (3) or more years of relevant experience in a leadership or supervisory role.
Previous employment in a healthcare or hospitality setting considered an asset.
Experience providing care plans and managing complex resident needs.

Knowledge, Skills and Abilities

In-depth knowledge of nursing best practices and diagnosis.
Knowledgeable of geriatric principles and elder care practices, including medication administration and monitoring.
Excellent written and verbal communication skills.
Demonstrated leadership skills with the ability to delegate tasks and motivate others.
Ability to apply relevant clinical skills including taking vitals, wound care, administering medications, assisting with mobility and daily living.
Strong documentation and charting skills.
Ability to remain calm and professional in stressful or emergency situations.
Ability to work in a team-based environment (e.g. staff, PSWs, RNs, attending physician, residents and family members).
Ability to meet the physical demands of the position, including lifting, transferring and supporting residents.
Intermediate proficiency with computer systems, including Microsoft Office suite (Word, Excel, Outlook, etc.).
