JOB SUMMARY
Siloam Mission’s Medical Transitional Care Unit (MTCU) is a compassionate 32-bed facility supporting individuals experiencing homelessness who have medical needs that do not require hospitalization but exceed what can be safely managed in shelter or in the community. The MTCU provides time-limited transitional housing integrated with essential medical, mental health, and social supports to improve health stability and housing outcomes.
Guided by person-centered, strengths-based, trauma-informed practice, and rooted in harm reduction and Housing First principles, the MTCU operates through a multidisciplinary team approach.
The MTCU Nurse Practitioner (NP) provides hands-on, comprehensive medical care within a medically complex transitional housing environment serving individuals with significant physical health needs, mental health challenges, substance use, and histories of trauma and homelessness. This role supports patients who may be actively using substances, experiencing untreated or unstable mental illness, and facing substantial barriers to accessing primary care. The role serves a diverse and highly vulnerable population and is grounded in principles of cultural safety, humility and respect. The NP delivers holistic, client-centered care that acknowledges the impact of colonization, intergenerational trauma, systemic racism, and social inequities. The NP works within a harm reduction and trauma-informed framework to manage acute and chronic medical conditions, reduce preventable emergency department visits and prolonged hospital stays, and support safe transitions to community-based care.
KEY DUTIES & RESPONSIBILITIES

Provide daily medical oversight, primary care and clinical support to residents in the MTCU, including daily rounds, monitoring health status, treatment adherence, and response to care

Engage in relationship-based care that builds trust and reduces barriers to accessing health services

Conduct comprehensive medical assessments, diagnosis, and treatment for patients with acute, subacute, and chronic conditions, including those with complex comorbidities

Deliver direct clinical care, including but not limited to:

Wound assessments and dressing changes
-
Injections and medication administration
-
Prescribing and medication management
-
IV antibiotic administration and monitoring
-
Acute care assessments and interventions
-
Basic Life Support (BLS) and medical response to urgent situations
-
Assess and respond to mental health challenges and behavioral concerns, including agitation, distress, and crisis situations, using trauma-informed and de-escalation approaches

Prescribe medications and order diagnostic tests in accordance with Manitoba scope of practice and regulatory standards
-
Provide care using a harm reduction, trauma-informed, and nonjudgmental approach, including working with individuals who are actively using substances
-
Manage complex medical conditions commonly associated with homelessness, substance use, mental illness, and limited access to healthcare
-
Initiate and manage treatment plans aimed at preventing prolonged hospitalizations and reducing avoidable emergency department utilization

Order, interpret, and act on diagnostic tests and clinical findings within scope of practice

Collaborate closely with interdisciplinary team members (e.g., healthcare attendants, nurses, case managers, occupational therapists, cultural and spiritual care supports, and physicians)

Support care transitions, including coordination with hospitals, primary care providers, specialty services, pharmacies, shelters, and community-based supports

Provide patient education related to health maintenance, symptom management, safer substance use practices, infection prevention, and self-care

Maintain accurate, timely clinical documentation in accordance with regulatory, legal, and organizational standards

Oversee practicum students in medical fields

Participate in quality improvement initiatives, clinical reviews, and program development as required

Respond to medical emergencies within the shelter environment as required
EDUCATION & EXPERIENCE
-
Master’s or Doctoral degree in Nursing with a Nurse Practitioner designation
-
Current Nurse Practitioner licensure in Manitoba
-
Active registration and in good standing with the College of Registered Nurses of Manitoba (CRNM)
-
Current BLS certification (ACLS an asset)

Experience working with substance use disorders and addictions, including harm reduction approaches (strong asset)
-
Experience providing mental health care, including assessment and management of common psychiatric conditions (strong asset)
-
Experience working with Indigenous peoples, communities, or Indigenous-led organizations (strong asset)
-
Experience working with diverse patient populations (strong asset)
-
Knowledge of Indigenous cultural safety, anti-racism, and TRC Calls to Action as they relate to health care delivery

Experience working with underserved populations strongly preferred

Demonstrated commitment to cultural humility, reflective practice, and ongoing learning

Excellent organizational, documentation, and time-management skills

Strong understanding of homelessness, poverty, and systemic barriers to care

Knowledge of Indigenous history, colonization, and culturally safe practices

Ability to work respectfully with and value diverse staff, including those with lived experience

Experience/Proficiency with Accuro EMR as a strong asset

Proficiency in Microsoft Word, Excel, and Outlook
RELATIONSHIP
-
Reports to Director of Community Wellness

Provides care to residents of the MTCU and individuals accessing services at Siloam Mission
-
Member of an interdisciplinary healthcare and housing support team
-
Acts as a liaison between Siloam Mission, hospital partners, and community service providers

Collaborates with Housing Leadership and Health Centre Leadership

Provides reports to senior management, funders, and the Board as required
WORKING CONDITION
-
Hours: Monday to Friday 8:00 a.m. – 4:30 p.m.; some evenings and weekends may be required
-
Work occurs within a transitional housing facility, including resident rooms, and within the Saul Sair Health Centre (SSHC) located within an Emergency Shelter

May be required to work independently with residents in their living environment
-
May be required to lift up to 50lbs, repetitive motions (bending, twisting, lifting, etc.)

Regular exposure to substance use, mental health crises, complex medical needs, and emotionally demanding situations
-
Potential exposure to unpredictable or risky behaviors, requiring situational awareness and adherence to safety protocols
WORKPLACE SAFETY AND HEALTH

Provides a safe environment by ensuring adherence to Workplace Safety and Health Regulations and Policies, Infection Control Guidelines, WHMIS and Safe Work Procedures

Demonstrates understanding of role and responsibilities in fire prevention and disaster preparedness and participates in safety and health training programs including the facility’s Fire, Disaster and Evacuation Plan

Salary: $136,364.80 - $169,540.80
