Bruyère Health Saint-Louis Long-Term Care is a 198 bed Long-Term Care (LTC) home with convalescent care, peritoneal dialysis and respite programs situated in a quiet residential neighborhood on a picturesque setting on the edge of the Ottawa river. What would my role be?
Reporting to the Manager, Resident Experience and Quality, Long-Term Care, you will be responsible for providing assessment, treatment, and evaluation of individual, interpersonal, and societal problems through the use of social service work knowledge, skills, interventions and strategies, to assist resident and their loved ones, to achieve optimum social functioning.
Your tasks will include:
- Provide supports to the residents and loved ones through their Collaborative Living Journey –from move-in to the time of their discharge or passing.
- Provide support to the resident, including through loss of function, ability, roles, and lifestyle changes, while supporting loved ones in the resident’s life and assisting families and loved ones in advocating for needs.
- Focus on enhancing the resident’s well-being and experience by promoting and respecting the dignity and individuality of each resident in order to support quality of life and reduce suffering.
- Coordinate all aspects of care conferences (including facilitating the care conferences).
- Promote continuity of care for residents of long-term care through the identification and use of services available in the community in collaboration with Ontario Health atHome or community services, thereby promoting the most beneficial placement for the resident.
How do I qualify?
Required:
- Two (2) year Community college diploma in Social Service Worker from an accredited college institution.
- Registered with the Ontario College of Social Workers and Social Service Workers.
- Demonstrate relevant experience with the elderly, behavioral disorders, palliative care, or convalescent care.
- Fluent in French and English (Candidates who do not meet this language requirement are eligible to apply for this position and may be considered if there are no other suitable applicants who meet this requirement).
Preferred:
- One (1) year of experience in long term care and gerontology.

- Demonstrated problem-solving skills, crisis intervention skills, time management skills
- Able to participate actively and effectively as a member of the interdisciplinary team.
- Able to facilitate care conference meetings and other sessions.
- Able to coordinate processes involving multiple team members and other stakeholders.
- Extensive knowledge of community resources
- Able to navigate Home and Community Care’s Health Partner Gateway portal, PointClickCare and other software/websites
- Demonstrated a good knowledge of issues encountered by persons with some form of dementia or physical challenges
- Intermediate Computer skills
- Basic knowledge of medical terminology
