Job Description

Job Title: Short Term Stay Housing Case Manager

Program: Early Intervention

Department: Men’s Emergency Services

Salary: $26.37 - $29.68 per hour

Reports to: Manager, Early Intervention

Jobs Reporting: N/A

Time Commitment: The STS Housing Case Manager will be required to work 35.7 hours per week with the ability to work day, afternoon, evening and weekend shifts in order to support staffing and program needs within a 24/7 setting.

Objective:

Following Housing First principles and as part of Mission Services’ Emergency Shelter Services team, the Housing Case Manager will provide homelessness case management support to individuals working to secure permanent accommodations while participating in our short-term transitional support program. The Housing Case Manager will work as a part of the Men’s Emergency Shelter Services team and will engage regularly with their clients and their colleagues to provide the necessary support to achieve program and participant goals.

Responsibilities and Duties:

Actively engage and interact with residents of the Short Term Stay Residence. Acting as part of the Men’s Services Case Management team and participating as the primary staff person in the development and implementation of individualized case plans
Work collaboratively with Men’s Services Emergency Shelter staff to develop and enhance housing case management plans
Maintain security of residents and buildings on all shifts
Respond to crisis situations involving service users by assessing the situation and implementing appropriate solution which promotes service user safety
Actively collaborate with the Facility Services teams’ in maintaining individual rooms
Actively participate in light lifting and cleaning to maintain rooms to ensure spaces are move in ready
Review and receive referrals internally and externally. Case conference on received referrals
Coordinate screenings for perspective residents and evaluate suitability for programming.
Coordinate resident payments for program fees, track program fees with residents and Mission Services Finance Department
Develop re-payment plans and budgeting for individuals unable to follow program fee deadlines
Coordinate resident move- ins and resident move outs.
Perform essential admission, and ongoing documentation and discharge duties, e.g., intake, documentation and records and reporting
Communicate with members of the case management team on issues regarding residents’ stays
Communicate regularly with manager, team members and a variety of community partners
Maintains a high level of familiarity with legislation, community services, resources, programs, policies and procedures, and issues affecting or related to the service user population
Provide direction to volunteers, students and new staff members providing support and instruction to ensure policies and procedures are followed
Other duties as required to ensure the continuation of service to residents/clients as it relates to achievement of individuals’ successful housing outcomes
Work safely, identify any health and safety concerns and report to management, and follow Mission Services workplace health and safety policies and procedures at all times

Qualifications:

Education

Post-secondary diploma or degree in human or social services, or equivalent experience
Preference for a certification in non-violent crisis intervention and First Aid/CPR
Experience

One to two (1-2) years of experience in Emergency Shelter work
Experience working with homeless adult males with mental health and addictions issues, providing non-judgmental supports to them
Experience in case management techniques, use of and connection to community resources
Knowledge

Must have a knowledge of community resources available for service users including housing providers, income maintenance programs, shelters and other service providers
Must have knowledge of relevant legislation, program policies & procedures
Skills

Excellent interpersonal and diplomacy skills
Excellent computer, oral and written communication
Must be detail oriented
Abilities

Ability to form helping relationships based on trust and empowerment
Ability to take initiative
Ability to determine the needs of an individual
Ability to maintain a calm demeanor when dealing with sensitive issues within a stressful work environment
Ability to work independently and part of a team
Other Qualifications

Proof of negative 2-Step Tuberculosis Testing
Proof of clear Police Check with Vulnerable Sector Check

Nature and Scope:

Contacts

Frequent contact with service users
Frequent contact with Men’s Shelter management and other Men’s Services staff
Occasional contact with various sources outside of mission services
Level of Responsibility

Assess service user mental health needs to determine shelter suitability
Checks belongings for weapons
Monitors service users for intoxication and provide first aid or contact emergency services as needed
Use of petty cash for minor shelter items
Enforcing policies and guidelines
Responsible for collecting, maintaining and recording confidential data

Working Environment

Mainly working in shelter environment
Some traveling required
Possibility of encountering and responding to unpredictable or crisis situations
