Community Health Worker Full-time, 1 Year Contract (35 hours per week) $56,450 to $66,871 per annum plus benefits (salary commensurate on experience)(new position)

About Unison:

Unison Health and Community Services is a non-profit, community-based organization that serves neighbourhoods in North West Toronto. Operating out of six locations, Unison offers core services that include primary health care, counselling, health promotion, Early Years programs, legal services, harm reduction programs, housing assistance, adult protective services, Pathways to Education™ and a Diabetes Education Program. Unison’s mission is working together to deliver accessible and high quality health and community services that are integrated, respond to needs, build on strengths and inspire change.

The Community Health Worker will play a leading role in developing, implementing and helping to evaluate community-based programs and initiatives across York–Weston–Pelham communities. Working in alignment with the vision, mission, and values of Unison, this position will collaborate closely with the York–Weston–Pelham Community Cluster and focus on supporting the unique needs and priorities of the local community. This position is accountable to the Senior Director, Community Health and Quality.

Job-Specific Responsibilities:

· Identify community health needs, and develop, implement and help evaluate evidence-informed community-based programs and initiatives responsive to local priorities

· Support community development by participating in community-based networks, fostering collaborative partnerships, engaging community leaders, supporting leadership development, and serving as a resource to Unison and partner agency staff

· Build and maintain accurate information of community health and social service programs and agencies to facilitate effective referrals and coordinated client support

· Work collaboratively and flexibly with partner agencies, adapting to diverse organizational practices and developing an understanding of each agency’s role, services, and community approach

· Have an understanding of laws, acts and legislation that impact marginalized communities

· Advocate for clients and community partners to reduce barriers to services and promote equitable access and the social determinants of health

· Provide leadership for promotion of and education about Unison’s services and programs in the community, with a focus on connection of unattached people to primary care, and on mental health programs and services

· Document and share client experiences, challenges, and successes to support continuous improvement in client care and service delivery

· Monitor, evaluate and report on program activities, trends, barriers, outcomes and community impact.

Job-Specific Qualifications:

· Undergraduate degree or college diploma in social services, health promotion or related disciplines with previous experience in health, social service or related field, or equivalent experience

· Extensive experience in leading, developing and helping to evaluate health promotion programs and initiatives

· Extensive experience with assessing individual client needs and making referrals

· 3-5 years experience working in a community-based setting

· Experience serving the area’s priority populations (seniors with complex needs, people with mental health and substance use challenges, LGBTQIA2+, youth facing barriers, people with lower incomes, and people with lived experience of racism)

· Demonstrated knowledge of, and sensitivity to the impact of social, economic, environmental and cultural issues on diverse communities

· An understanding of local neighbourhoods, issues and priorities

· Experience working with local service providers, networks and community groups.

· Knowledge of issues affecting communities facing barriers to access in an urban environment including:

a) Community/social issues

b) Community development

c) Advocacy

d) Discrimination

e) Determinants of health

f) Chronic disease prevention

g) The various resources available to community

· Excellent interpersonal, communication and organizational skills, including computer literacy, particularly spreadsheets and databases

· Excellent communication skills, both written and verbal, with individuals and groups

· Knowledge of a second (or more) language and culture is an asset

· Availability to work flexible hours including some evenings and weekends
