What We are Looking For:
We are currently recruiting for a Nurse Practitioner to join our Homeless and Addiction Recovery Treatment team (HART Hub) for a part-time position. This is a new role and the successful candidate would be responsible for providing primary care functions and participating in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the community. This position works directly with outreach clients experiencing homelessness or precarious housing and provides support to mobile primary care teams across Cambridge & North Dumfries.
A successful candidate in this role would have
· Baccalaureate in Nursing, Primary Health Care Nurse Practitioner Certificate
· College of Nurses of Ontario Certificate of Competence with RN (EC) designation (unrestricted)
· Additional academic preparation in health or social sciences an attribute
· Commitment to the use of determinants of health model of care
· A strong understanding of local community resources
· Minimum 2 years’ experience as an NP in a primary care setting
· Experience working with individuals and families in all developmental stages
· Experience working in an outreach setting and serving those with substance use or addiction issues an asset
· Experience working as a member of a multidisciplinary team
· Experience in community development, prevention programming and health promotion
· Experience with Chronic Disease Management
· Experience in program development, implementation, monitoring and evaluation an asset
· Experience working with individual and group members of diverse populations
· Excellent communication and organizational skills
· Proficiency in the use of computers and various software applications
· Ability to work one evening (12:00pm-8:00pm) per week
· Valid G-Class license and ability to travel throughout the Cambridge and ND area to outreach sites and home visits
In this role you will be expected to complete the following tasks
· Provides primary care functions to vulnerable individual’s experiencing increased barriers to health care services.
· Perform assessment, diagnosis, treatment, health education/counselling and other clinical activities according to the College of Nurses of Ontario standards of practice for Registered Nurses in the Extended Class (NPs).
· Contribute to chart documentation, participate in chart reviews and case conferences, and make referrals.
· Provide addiction and mental health care imbedded in the primary care model for people with complex physical and mental health needs.
· Use office assessment, telephone advice, home visits, satellite, specialty clinics, partner offices, mobile van and a variety of other approaches in the delivery of primary care.
· Work with Langs, internal and external community partners to identify needs, plan, and conduct and evaluate programs including health teaching, screening programs and health promotion workshops for Langs patients/participants and other community groups that specifically outreach to vulnerable sector individuals.
· Apply and participate in the development of appropriate professional and organizational policies, protocols and procedures.
· Understand the role of, refer appropriately to and communicate effectively with other health professionals and community services.
· Participate in the development and maintenance of Langs health resource materials.
· To perform other associated duties as required.

The successful candidate would be eligible for the following benefits pro-rated to the hours of work:
· Hourly rate $52.89-$63.02
· 3 Weeks paid vacation, with an increase to 4 weeks after 1 year of employment
· 15 Sick/Personal Days per year
· 10 Professional Development days per year
· Paid 30 minute lunch break
· Enrolment in Healthcare of Ontario Pension Plan (HOOPP)
