Primary Care Nurse Practitioner – Multiple Opportunities

Prosperity Workforce Solutions is hiring Primary Care Nurse Practitioners on behalf of a well-established community health organization serving diverse and underserved populations across Scarborough.
Multiple opportunities are available across community primary care, youth wellness, mental health & addictions, and geriatrics/long-term care within a collaborative, interdisciplinary care model.
These roles are ideal for Nurse Practitioners who are passionate about equitable healthcare, adaptable in dynamic environments, and committed to supporting patients with complex medical, mental health, and social needs.
Nurse Practitioners will provide comprehensive primary care services with a strong emphasis on:
- Health promotion and disease prevention
- Chronic disease management
- Mental health and substance use care
- Care coordination and system navigation
- Supporting underserved and marginalized populations
This is not a traditional clinic-only NP role. In addition to direct clinical care, NPs play an important role in care navigation, referrals, advocacy, outreach, and interdisciplinary collaboration to improve patient access and outcomes.
Care is delivered across a variety of settings, including:
- Community health clinics
- Youth Wellness Hubs (YWHO model)
- Long-term care and geriatric programs
- Community outreach sites
- Patient homes
- Mobile/community care settings
Mobile and outreach care are integrated into all positions to improve access for patients facing barriers to traditional clinic-based services.
Work Model & Travel Expectations
This is a hub-based model operating across multiple Scarborough locations.
Nurse Practitioners are assigned a primary hub and manager but may be scheduled to work across other community sites, outreach settings, or patient homes based on operational needs and patient access priorities.
Travel may occur using:
- Personal vehicle
- Mobile clinic van
All work locations are within Scarborough. Travel time between sites is paid.
Travel between sites typically occurs approximately once per week or more depending on rotation and assignment.
Schedules are established in advance, and NPs are informed ahead of time regarding location assignments and rotations.
Schedule & Flexibility Requirements
Candidates must be flexible and available to work a rotating schedule that may include:
- Days
- Evenings
- Weekends
- Community outreach shifts
Full-Time Schedule Examples
- Days: 9:00 AM – 5:00 PM
- Evenings: 12:00 PM – 8:00 PM
- Saturdays: 9:00 AM – 5:00 PM
Part-Time Schedule Examples
- Evenings: Approximately 4:00 PM – 8:00 PM
- Saturdays: 9:00 AM – 5:00 PM
On-Call Expectations
Participation in a scheduled on-call rotation is required for all Nurse Practitioners.
On-call responsibilities support multiple program sites and include:
- Responding to urgent clinical calls
- Escalating concerns appropriately
- Collaborating with backup NP and physician support
Additional compensation includes:
- On-call stipend
- Additional pay if called in
Key Responsibilities
- Provide comprehensive primary care within NP scope of practice
- Assess, diagnose, and manage acute and chronic conditions
- Deliver preventive care including immunizations and screening
- Develop and implement patient-centered care plans
- Manage complex cases involving mental health, addictions, and social determinants of health
- Prescribe medications and order/interpret diagnostics
- Perform primary care procedures (e.g., injections, Pap tests)
- Deliver care in clinic, outreach, community, and mobile settings
- Coordinate referrals and connect patients with internal and external supports
- Collaborate within an interdisciplinary care team including physicians, nurses, social workers, and allied health professionals
- Participate in case conferences and care planning discussions
- Support patient advocacy, education, and system navigation
- Maintain accurate clinical documentation
- Participate in quality improvement initiatives and team-based care planning
Patient Population
NPs will support diverse and underserved communities with varying levels of medical, mental health, and social complexity.
Patient presentations may include:
- Complex chronic disease
- Mental health and addictions
- Housing and social instability
- Significant psychosocial barriers to care
Candidates must be comfortable working with high-complexity patients and navigating situations that may require crisis escalation, urgent referrals, or coordination with community supports.
Specialty Focus Opportunities
NPs may have the opportunity to work across one or more of the following areas:
General Primary Care
Longitudinal, family-centered care for unattached and medically complex patients.
Youth Wellness (Ages 12–25)
Integrated youth-focused care addressing mental health, substance use, and low-barrier access within a YWHO model.
Geriatrics & Long-Term Care
Care for aging populations with complex chronic disease and geriatric care needs.
Qualifications
- Master’s Degree in Nursing with Primary Health Care Nurse Practitioner certification
- Registered and in good standing with the College of Nurses of Ontario (CNO) – Extended Class
- Valid CPR certification
- Valid driver’s license and access to a vehicle
- Strong clinical assessment and independent decision-making skills
- Ability to work effectively within a community-based and interdisciplinary care model
- Strong adaptability, critical thinking, and resourcefulness
