Job Title: Registered Practical Nurse – Community & Clinic

About us

Victoria Medical Wellness is an integrated community health and pharmacy practice in Kitchener, serving a complex patient population that includes individuals in supportive housing, shelter settings, and an on-site physician clinic. We work at the intersection of harm reduction, primary care, and pharmacy — and we take that work seriously.

About the role

We're looking for a self-directed RPN who thrives with minimal supervision and moves efficiently in a fast-paced environment. This role combines community outreach with clinic-based support. It starts as a part-time morning contract — approximately 20-30 hours per week — with real potential for increased hours and a transition to permanent employment for the right person.

What you'll do

Travel a daily morning route to partner shelter and supportive housing sites to support medication administration for patients in addiction treatment programs
Provide clinical and administrative support for clinic patients — intake, monitoring, follow-up coordination
Perform point-of-care testing (POCT) as directed
Administer injections within RPN scope of practice
Complete clinical documentation accurately and on time
Communicate effectively with pharmacists, prescribers, and community site staff

What we're looking for

Current RPN registration in good standing with the College of Nurses of Ontario
Valid Ontario driver's licence and reliable personal vehicle — travel between sites is required daily
Ability to work independently and manage your own time with minimal oversight
Efficient and organized in a high-volume, community-facing environment
Strong interpersonal skills — you'll work with individuals facing complex social and health challenges and must bring patience, discretion, and professionalism
Experience in harm reduction, addiction medicine, or community nursing is a strong asset

Details

Starts as a part-time contract — approximately 20-30 hours per week, generally mornings
Potential to grow into increased hours and permanent employment based on fit and operational needs
Sites include shelter, supportive housing, and clinic locations across Kitchener
Competitive hourly rate commensurate with experience
