The Opportunity

Loon River First Nation is seeking a dedicated and compassionate Patient Navigator to join our Health team. Reporting directly to the Health Director, the Patient Navigator plays a vital role in supporting community members as they access and navigate Alberta’s health care system.

This position focuses on advocacy, care coordination, and culturally safe support to ensure members receive respectful, equitable, and timely health services. The Patient Navigator works closely with Alberta Health Services (AHS), hospitals, specialists, and community providers to reduce barriers to care and improve overall health outcomes.

Who You Are

As our Patient Navigator, you are empathetic, organized, and culturally grounded. You understand the complexities of the healthcare system and are passionate about supporting First Nations members in receiving dignified and equitable care. You build trust easily and advocate confidently on behalf of clients and families.

Your responsibilities will include:

Providing health system navigation, advocacy, and support for members accessing local and regional healthcare services
Assisting with appointment coordination, referrals, discharge planning, and follow-up care
Connecting unattached patients with primary care providers, mental health supports, allied health professionals, and community programs
Advocating for patient rights and addressing experiences of racism or discrimination within the healthcare system
Liaising with Alberta Health Services, hospitals, and care providers to ensure culturally safe and respectful treatment
Supporting hospital discharge planning and aftercare coordination
Promoting health literacy through community outreach, education, and culturally appropriate resources
Identifying service gaps and barriers to access within the health system
Developing and distributing resource guides and informational materials
Maintaining accurate and confidential client records

Qualifications

To thrive in this role, you possess the following:

A diploma or relevant experience in health navigation, social work, community health, or a related field
Experience working within First Nations communities or Indigenous organizations
Understanding of Alberta’s healthcare system, including primary care, public health, and Indigenous health programs
Knowledge of trauma-informed care and the impacts of colonialism and intergenerational trauma
Strong communication, organizational, and advocacy skills
Proficiency in Microsoft 365 (Outlook, Word, Excel)
Valid Driver’s License and access to reliable transportation
Knowledge of Cree language, culture, and traditions is a strong asset
Clear Criminal Record Check and Vulnerable Sector Check
