Salary Range: $56,444 to $68,429 per annum. *Candidate experience will be considered when determining the starting salary.
Position: Contract, full-time, working 35 hours a week. Contract ending March 31, 2028. Occasional evenings and weekends may be required.

We are hiring a Case Manager in our Housing First! This individual will be considered a housing specialist for the team!
As we continue to grow our team, we are looking for a Case Manager. If you are a collaborative, innovative individual who is interested in helping people recover from a mental health barrier, increase their independence, and find housing in York Region and South Simcoe, then this role is for you!

Location: The successful candidate be required to work out of the Aurora Office located at 15150 Yonge Street, Aurora, Ontario, and will be required to travel extensively within York Region & South Simcoe for client meetings, staff training and meetings.
Job Profile:
The Case Manager works within a Housing First model and collaborates with clients to provide individualized and flexible support to help them find housing, engage and integrate them into the community, helps to facilitate their access to resources and supports to maximize their independence and potential, and promote their recovery. The work with clients is achieved with a focus on Housing stabilization.
Individuals are assisted to identify strengths and needs and develop long term goals, short term objectives to meet these goals, and concrete action steps to achieve the objectives. In addition, the Case Manager participates in individual and systemic advocacy initiatives to improve resource access and availability. The Case Manager may be assigned other duties as required.
In addition to compliance with all organizational policies and procedures, the incumbent is also expected to behave ethically, demonstrate competence in effective communication and team building, demonstrate effectiveness in working in a collaborative environment, and demonstrate confidence in decision-making and in building relationships.
The Case Manager may be assigned to different programs as required based on need (i.e.: Flexible Support, ACTT, Case Management, etc.)
Responsibilities:
- Adheres to the Intensive Case Management Services Standards of the Ontario Ministry of Health and Long-Term Care.
- Maintains a client caseload in accordance with the above standards.
- Is familiar with signs and symptoms of trauma in clients and is able to respond appropriately in making referrals and providing short-term assistance to clients to help them to manage these symptoms.
- Performs comprehensive assessments.
- Initiates referrals to other CMHA programs or external agencies as required.
- Anticipates, understands, and responds to the needs of internal and external clients within organizational parameters in order to meet or exceed their expectations.
- Establishes a collaborative partnership with clients that involves non-judgmental listening and client-directed goal setting that fosters independence, self-determination, competence and hope, by employing empowerment/Recovery principles and practices.
- Uses effective listening skills to address the clients’ issues and needs.
- Informs clients of confidentiality requirements and limitations of confidentiality.
Requirements:
- Degree with four years’ experience or Diploma with six years’ experience in a community based mental health organization; experience in homelessness sector is preferred.
- Current registration with OCSWSSW or a regulatory body under the Regulated Health Professions Act. Cannot be in the qualifying stages.
- A second language (French, Cantonese, Farsi, Italian, Korean, Mandarin, Punjabi, Russian, Tamil, Urdu) reflecting the local community is an asset.
- Knowledge of the Ontario Mental Health Act, Residency Tenant Act, mental health reform principles, the Substitute Decisions Act, the Health Care Consent Act and PHIPA requirements.
- Ability to apply Recovery principles and empowerment-oriented philosophies and practices in work with consumers.
- Extensive knowledge of supports and services in York Region and South Simcoe, including formal and informal resources.
- Knowledge of systemic issues such as poverty, unemployment, stigma, medications and the isolation felt by individuals with serious mental health issues and their families.
- Ability to observe boundaries, engage in appropriate emotional regulation, refrain from dual relationships with consumers, maintain confidentiality, and engage in reasonable self-care strategies designed to reduce stress by balancing work/life responsibilities.
- Excellent oral and written communication skills
- Ability to demonstrate computer skills, specifically Microsoft Office and Outlook
- A valid Canadian Full G driver’s license, a minimum $1,000,000 third party liability insurance
- A satisfactory and current Vulnerable Sector Screening (Police Check).
