Full job description

Job Overview
We are seeking a compassionate and dedicated Registered Practical Nurse (RPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role requires a professional with strong clinical skills, excellent communication abilities, and a commitment to improving patient outcomes. The RPN will work collaboratively with multidisciplinary teams to deliver comprehensive nursing services tailored to individual patient needs.

Duties

Administer medications and treatments accurately following physician orders and established protocols
Monitor patient vital signs and assess health status regularly
Assist with patient mobility, hygiene, and comfort measures
Document patient care activities using electronic health records such as Epic and Cerner systems
Provide specialized care for patients with dementia, seniors, pediatrics, or those requiring hospice or pain management services
Conduct phlebotomy procedures and perform laboratory specimen collection as needed
Educate patients and families on health conditions, medication management, and discharge instructions
Collaborate with physicians, therapists, and other healthcare professionals to develop and implement care plans
Ensure compliance with all safety standards, infection control protocols, and regulatory requirements
Participate in case management activities to coordinate ongoing patient care

Requirements

Current registration as a Registered Practical Nurse (RPN) with applicable licensing body
Proven experience in nursing home, hospital, acute care, or home care settings preferred
Knowledge of medical terminology, medication administration, and clinical procedures such as phlebotomy
Experience working with diverse patient populations including seniors, children, or hospice patients is advantageous
Familiarity with electronic health record systems like Epic or Cerner is preferred
Strong communication skills and ability to work effectively within a multidisciplinary team
Ability to handle sensitive situations with professionalism and compassion
Certification in pain management or case management is a plus
Previous experience in dementia care, assisted living, or laboratory procedures is beneficial
