About the role:
The Northern York South Simcoe Ontario Health Team is seeking a highly organized, collaborative, and implementation focused leader to support the advancement of the Neighbourhood Health Home model across the NYSS OHT geography. The vision for a Neighbourhood Health Home is an integrated, coordinated network of care providers with collective accountability to provide guaranteed attachment and access to primary care services to every resident of a well-defined geography. The model is intended to strengthen access to team-based primary care, improve attachment pathways for unattached residents, and better connect primary care, interprofessional teams, hospitals, community support services, mental health and addictions, home and community care, public health, paramedicine, and other OHT partners around local population needs.
This role will help move the model from planning into implementation by developing practical workflows, coordinating partner engagement, supporting change management, aligning workstreams to OHT priorities and Ontario Health deliverables, and ensuring the model is operationalized in a way that is clear, measurable, sustainable, and achievable. The successful candidate will be comfortable working in a complex, multi-partner environment where success depends on relationship-building, practical problem-solving, disciplined project management, and the ability to bring structure to emerging work.
What you'll be doing:
Neighbourhood Health Home Implementation
- Support the development and communication of the Neighbourhood Health Home vision
- Support the implementation of the Neighbourhood Health Home model across priority communities, beginning with identified initial geographies and expanding over time.
- Translate the Neighbourhood Health Home vision into a practical implementation plan, including milestones, accountabilities, timelines, risks, and decision points.
- Support the development of hub-and-spoke operating models that connect IPCTs, primary care practices, community providers, hospitals, and OHT partners.
- Develop and document workflows, standard operating procedures, and escalation pathways related to patient attachment, care navigation, referral pathways, and partner integration.
- Help identify which services should be delivered directly through the Neighbourhood Health Home model and which should be accessed through existing OHT partners to avoid duplication.
Alignment with OHT Priorities and TPA Deliverables
- Map Neighbourhood Health Home activities to the NYSS OHT work plan, strategic priorities, and Ontario Health TPA deliverables.
- Ensure implementation activities support key areas such as primary care access and attachment, supported attachment, chronic disease prevention and management, ALC/Home First, cancer screening pathways, care navigation, and population health planning.
- Support the development of clear deliverables, outputs, and performance indicators that demonstrate progress without overcommitting beyond the OHT's direct control.
- Work with OHT leadership to ensure the model is positioned as the operating structure to advance existing priorities, rather than as a separate or competing initiative.
Partner and Primary Care Engagement
- Build and maintain strong relationships with primary care providers, PCN members, hospitals, community providers, and other OHT partners.
- Support engagement with physicians, nurse practitioners, interprofessional primary care teams, family health teams, community health centres, and other primary care partners.
- Coordinate meetings and working sessions with partners to define roles, responsibilities, workflows, and implementation requirements.
- Support the integration of OHT network tables and partner organizations into the Neighbourhood Health Home model in a staged and practical way.
- Help identify opportunities for partners to contribute directly to implementation, including care pathways, navigation supports, chronic disease management, patient education, and community-based services.
Access, Attachment, and Navigation
- Support the design and implementation of attachment pathways for unattached residents.
- Work with partners to define how residents can be connected to available primary care, interprofessional supports, screening, and community services.
- Support the development of navigation processes that allow patients to be connected to the right provider, program, or partner based on need.
- Help align attachment work with privacy, consent, data-sharing, and documentation requirements.
- Support short-term operational solutions while also helping to develop longer-term sustainable workflows and infrastructure.
Project Management and Change Leadership
- Lead or support implementation workstreams, including project charters, workplans, timelines, risk logs, issue tracking, and status reporting.
- Facilitate working sessions with internal teams and external partners to move decisions forward.
- Identify dependencies, barriers, risks, and resource needs early, and escalate issues as appropriate.
- Support change management by helping partners understand the model, their role, and the practical changes required to implement it.
- Bring structure, clarity, and momentum to complex work involving multiple organizations and stakeholders.
Measurement, Reporting, and Continuous Improvement
- Work with OHT leadership and partners to define practical measures of progress and impact
- Support reporting related to implementation milestones, attachment pathways, partner participation, workflow development, and service integration.
- Help ensure measurement is meaningful, feasible, and aligned with Ontario Health expectations.
- Support continuous improvement by identifying what is working, what requires adjustment, and where additional partner support is needed.
- Contribute to briefing notes, presentations, updates, and reports for OHT leadership, governance tables, partner meetings, and Ontario Health as required.
What you bring:
- Undergraduate degree in health administration, public health, business administration, social sciences, health sciences, or a related field.
- Master's degree in health administration, public health, business administration, quality improvement, or a related discipline is preferred.
- Minimum 5 years of experience in health system planning, project management, implementation, change management, health care operations, or integrated care delivery.
- Experience working in Ontario's health care system, preferably with primary care, OHTs, hospitals, community care, home and community care, mental health and addictions, or interprofessional team-based care.
- Demonstrated experience supporting complex, multi-stakeholder initiatives from planning through implementation.
- Experience developing workplans, workflows, standard operating procedures, briefing materials, reports, and implementation tools.
- Experience with performance measurement, project reporting, risk management, and stakeholder engagement.
- Knowledge of Ontario Health Teams, primary care access and attachment, integrated care models, and population health planning is considered an asset.
- Project management, change management, Lean, quality improvement, or equivalent certification is considered an asset.
- Strong project management and implementation skills.
- Ability to translate strategy into practical operational plans.
- Excellent facilitation, communication, and relationship-building skills.
- Strong judgment and ability to work through ambiguity.
- Ability to engage physicians, clinical leaders, administrators, community partners, and senior leaders.
- Strong writing skills, including the ability to prepare clear briefing notes, reports, presentations, and workplans.
- Ability to manage multiple priorities in a fast-paced and evolving environment.
- Practical understanding of privacy, consent, information-sharing, and health system accountability considerations.
- Collaborative, solutions-focused, and comfortable working across organizational boundaries.
