Mobile Fitness Facilitator

TNO - The Neighbourhood Organization (TNO) is a well-established community-based social and health service agency providing a wide range of programs and services supporting low-income and newcomer communities across Ontario and sometimes across different provinces through online services and referrals. We are seeking a Mobile Fitness Facilitator to join our team in a full-time permanent role.

The Mobile Fitness Facilitator plays a central role in assessing, designing, delivering and evaluating evidence‑based fitness, injury‑prevention, and rehabilitation programs for older adults and individuals with musculoskeletal conditions. This role combines functional fitness expertise with therapeutic exercise knowledge to support safe movement, improved mobility, and enhanced quality of life. The successful candidate will have a degree or diploma in Kinesiology, Sport Science, Exercise Science, Fitness & Health Promotion, Physiotherapy/Occupational Therapy Assistant, Recreation, or a related field, with a minimum of two years of experience delivering individual and group fitness programs for older adults and individuals with chronic conditions. The ideal candidate will possess strong knowledge of therapeutic exercise, functional movement, fall prevention, injury prevention, and healthy aging, with experience adapting exercise programs to meet diverse physical abilities and health needs. Certification or eligibility for registration with a recognized professional body (e.g., CSEP, OKA, or CanFitPro with therapeutic specialization) is preferred. The candidate will demonstrate excellent communication, interpersonal, organizational, and documentation skills, with the ability to work independently and collaboratively within an interdisciplinary team. A commitment to providing client-centred, culturally responsive, anti-oppressive, and inclusive services is essential.

DUTIES & RESPONSIBILITIES

Design, implement, and deliver safe, progressive, evidence-based fitness and therapeutic exercise programs for older adults through both individual and group sessions, adapting exercises to meet participants' abilities, health conditions, and goals.
Conduct comprehensive client assessments, identify health and mobility risks, develop individualized fitness and rehabilitation plans, and perform ongoing reassessments and program evaluations, including GLA baseline and post-program assessments.
Document treatment plans, attendance, progress and outcomes in EMR or as directed.
Design and deliver safe, progressive exercise programs
Provide one‑on‑one and group‑based therapeutic exercise sessions.
Educate clients on proper movement mechanics, joint protection strategies, and self‑management techniques.
Conduct pre‑program baseline assessments and post‑program evaluations for GLA:D participants together with GLA:D colleagues.
Co‑lead GLA:D exercise sessions following standardized protocols.
Track participant outcomes, adherence, and functional improvements.
Collaborate with physiotherapists or clinical leads to ensure program fidelity and quality.
Provide education sessions related to osteoarthritis, pain management, and evidence‑based exercise strategies.
Create a welcoming, supportive, and inclusive environment that encourages social interaction and motivation among participants.
Offer individualized guidance and encouragement during fitness sessions to enhance participant experience and results.
Address participants' questions, concerns, and feedback, demonstrating a commitment to their well-being and progress.
Deliver workshops or individual coaching.
Promote long‑term adherence to physical activity and self‑management practices.
Maintain exercise equipment and ensure the exercise Create a welcoming, supportive, and inclusive environment that encourages social interaction and motivation among participants.
Foster positive relationships with participants, understanding their goals, preferences, and limitations.
Offer individualized guidance and encouragement during fitness sessions to enhance participant experience and results.
Address participants' questions, concerns, and feedback, demonstrating a commitment to their well-being and progress.
Maintain current knowledge of evidence-based practices, industry trends, and clinical protocols related to senior fitness and chronic disease management, including osteoporosis, diabetes, and osteoarthritis.
Implement safety protocols and risk assessment measures to prevent injuries during fitness sessions.
Work collaboratively with physiotherapist, program facilitators, and healthcare partners.
Participate in interdisciplinary case discussions when required.
Contribute to program development, quality improvement, and community outreach initiatives.
Support TNO outreach activities.

Screening – A clear Vulnerable Sector Check (VSC) is required as a condition of employment and must be obtained prior to the successful candidate's start date.

Job Type: Full-Time permanent position.

Rate of Pay: $28.00 per hour.

Vacancy Status – This position is for an existing vacancy – replacement.

Work Location(s): 10 Gateway Blvd, 45 Overlea Blvd, other TNO sites, and clients’ homes depending on program needs.

Schedule: 35 hours per week between 8:30 AM to 8:00 PM besides evening shifts and weekends as decided by program and community requirements.
