We are currently looking to hire a:

Registered Dietitian

1 – Full Time, Permanent (35 hours per week)

Reporting To: Interprofessional Primary Care Manager

Purpose:

The Scarborough interprofessional primary care access model is a partnership to enhance care for the vulnerable, complex clients living in the Scarborough North and South Sub-regions. The priority population includes over 900 people currently on the Health Care Connect list, as well as non-insured and clients who require access to interprofessional teams.

Bring your passion for community-based healthcare to this face-paced and dynamic environment, as well as your program development and implementation experience to this role.

Key Areas of Responsibility

The Registered Dietitian (RD) provides comprehensive nutrition assessment, counseling, and health promotion services to clients with diverse health, social, and nutritional needs. The RD facilitates individualized nutrition care planning, ongoing monitoring, interprofessional collaboration, and referral coordination to support clients and their families. The RD assists clients in achieving improved health outcomes by delivering culturally responsive, evidence-informed nutrition services and programs within the IPPC team and in partnership with community service providers.

As a key member of the team, the Registered Dietitian will:

Clinical Nutrition Care

Conduct comprehensive nutritional assessments for individuals and families, particularly those who are marginalized, non-insured, experience food insecurity, or present with chronic or complex conditions.
Develop individualized nutrition care plans and provide 1:1 and group counseling using evidence-based practice and culturally safe approaches.
Support chronic disease management (e.g., diabetes, hypertension, obesity, GI conditions) in collaboration with NPs and other providers.
Provide trauma-informed, client-centered counseling adapted for diverse cultural backgrounds.

Program Development & Implementation

Develop, implement, and evaluate nutrition programs and group interventions that respond to community needs.
Support food security and health promotion initiatives in partnership with internal and external stakeholders.
Identify gaps in services and contribute to innovative solutions that improve nutrition access and overall well-being.

Interprofessional Collaboration

Participate in case conferences, care planning meetings, and integrated service delivery.
Provide clinical consultation to NPs, navigators, mental health professionals, and allied health staff.
Contribute to interdisciplinary care plans and ensure coordinated, seamless service for clients.

Community Outreach & Engagement

Deliver services across SCHC sites and in the community, including outreach clinics, partner organizations, and mobile service models.
Build relationships with community partners to improve food access, support newcomer populations, and address social determinants of health.

Equity, Cultural Safety & Advocacy

Embed anti-oppressive, trauma-informed, and culturally safe approaches in all service delivery.
Advocate for systemic changes that address barriers related to poverty, food security, race, ethnicity, immigration status, and health equity.

Documentation, Quality & Best Practice

Maintain accurate and timely documentation within PS Suites and other digital tools.
Contribute to quality improvement initiatives, data collection, and outcome measurement.
Stay current on evidence-informed nutrition practices relevant to primary care and community health.

Educational and/or Professional Qualifications

· Undergraduate degree in Food and Nutrition from an accredited university.

· Completion of a Dietitians of Canada–accredited internship (preferably with a community focus).

· Registration in good standing with the College of Dietitians of Ontario.

· Minimum five years of experience providing clinical nutrition services; community health and primary care experience are strongly preferred.

· Demonstrated experience in program development, implementation, monitoring, and evaluation.

· Strong knowledge of the social determinants of health impacting low-income, racialized, newcomer, and uninsured communities.

· Excellent counseling, communication, interpersonal, organizational, and time management skills.

· Ability to work independently with strong clinical judgment, while excelling in collaborative team environments.

· Proficiency working with electronic health records and digital tools (PS Suites experience as an asset).

· Second language relevant to Scarborough’s priority populations (e.g., Tamil, Urdu, Arabic, Farsi, Mandarin, Cantonese) is an asset.

· Current CPR/First Aid certification (or willingness to obtain).

· Knowledge of Ontario’s healthcare system, community health models, and food security frameworks.

Significant Working Conditions

Flexibility of hours, including occasional evenings and weekends, will be required.
Flexibility to work across multiple sites as needed.
Valid driver’s license and reliable vehicle required.
Fast-paced environment requiring adaptability, prioritization, and ability to balance competing demands.

· Perform other related duties as required to support client care, program delivery, and organizational priorities.

Remuneration: $35.05 – $42.22
