Full job description

Key Responsibilities:

Provide psychosocial assessment of community clients and their caregivers
Provide individual, family and group counselling to clients and their caregivers
Provide assessment and counselling to clients who are bereaved
Manage and maintain clinical files and related documentation in accordance with best practices and policies of Hospice Care Ottawa.
Backfill other team members in providing care across the hospice sites as required.
Keep and record monthly statistics electronically and as required.
Provide support and training for our volunteers
Liaise with community services/providers

Responsabilités principales :

Fournir une évaluation psychosociale des clients de la communauté et de leurs aidants
Fournir des services de counseling individuels, familiaux et de groupe aux clients et à leurs aidants
Fournir des services d'évaluation et de counseling aux clients en deuil
Gérer et tenir à jour les dossiers cliniques et la documentation connexe conformément aux meilleures pratiques et aux politiques de la Maison des soins palliatifs d’Ottawa
Remplacer d'autres membres de l'équipe dans la prestation de soins dans les différents sites de soins palliatifs, au besoin.
Tenir et enregistrer les statistiques mensuelles par voie électronique et selon les besoins.
Fournir un soutien et une formation à nos bénévoles.
Assurer la liaison avec les services/prestataires communautaires.

Registered Social Worker in Ontario
Bilingualism (French and English) oral and written is considered an asset
5 years experience as a Social Worker with at least 2 years of focus on grief and bereavement.
Strong communication skills, oral and written
Experience in group facilitation with bereaved clients
Possess the organizational skills to efficiently and successfully handle multiple tasks and competing priorities
Ability to work independently as well as work collaboratively within an interdisciplinary environment
Strong computer skills and experience in electronic documentation
Ability to work occasional evenings and weekends as needed
Travailleur social agréé en Ontario
Bilinguisme (français et anglais) oral et écrit est considéré comme un atout
Au moins deux ans d'expérience dans le domaine des soins palliatifs
Solides compétences en communication, à l'oral et à l'écrit
Posséder les compétences organisationnelles nécessaires pour gérer efficacement et avec succès plusieurs tâches et priorités concurrentes
Capacité à travailler de manière autonome et en collaboration dans un environnement interdisciplinaire
Solides compétences informatiques et expérience en matière de documentation électronique

The Family Support Counsellor is a member of the interdisciplinary Family Support team at Hospice Care Ottawa, and provides psycho-social, emotional and bereavement support counselling to clients and caregivers in our programs at HCO inclusive of Day Hospice, In Home Support, Residence, Caregivers and Bereaved Family/Friend

Home Base: May Court Hospice located at 114 Cameron Ave (Occasional travel and cross coverage at all three of our residence sites including Maison de L’Est at 380 LeBoutillier Ave. as required)

Union Affiliation: This role is an included position in the CUPE Local 2017 Collective Agreement.

Compensation: Starting at $42.58 an hour as outlined in the Collective Agreement. Eligible for Group Benefits (Health/Dental, Life Insurance & Disability) after 90 days. Eligible for paid personal/sick leave
Vacation: As per the CUPE Collective Agreement: Two weeks annually; however use of personal days and/or unpaid days is flexible

Le conseiller en soutien familial fait partie de l'équipe interdisciplinaire de soutien familial d'Hospice Care Ottawa et offre un accompagnement psychosocial, émotionnel et de soutien au deuil aux clients et aux aidants participant à nos programmes chez HCO, notamment l'hospice de jour, le soutien à domicile, la résidence, les aidants et les familles ou amis en deuil.

Lieu de travail : May Court Hospice, situé au 114, avenue Cameron (déplacements ponctuels et remplacement dans nos trois établissements, y compris la Maison de L’Est, au 380, avenue LeBoutillier, selon les besoins)
