Job Summary:
Portage Medical Family Health Team, is affiliated with a group family practice of 6 physicians dedicated to providing comprehensive healthcare, is seeking a passionate and dedicated Masters of Social Work/Psychotherapist to join our team. The ideal candidate will have a strong commitment to working with diverse populations and delivering therapeutic services in a clinical setting.

Responsibilities:
- Conduct individual and group therapy sessions to address patients' mental health needs.
- Collaborate with clients to develop personalized treatment plans and goals.
- Utilize medical terminology and research findings to enhance therapeutic interventions.
- Provide compassionate patient care and support within a clinical environment.
- Collect and analyze data to evaluate and monitor client progress effectively.
- Demonstrate experience in working with pediatric patients and individuals with special education requirements.
- Employ case management skills to coordinate comprehensive care services for patients.

Requirements:
- Bachelor's or Master's degree in Psychology, Counseling, Social Work, Psychotherapy, or a related field.
- Previous experience working with children and adolescents is advantageous.
- Proficiency in patient care practices and counseling techniques.
- Knowledge of medical terminology and proficient in data collection methods.
- Ability to manage complex cases and collaborate effectively with multidisciplinary teams.
