Remote Care Monitoring – Program Assistant

Temporary Full Time – 12 Month Contract (Hybrid)

Pay Rate: $27.00/HR to $32.00/HR

The Toronto Grace Health Centre (TGHC) provides, specialized care to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Transitional Care, and Palliative Care services. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 3 sites – the Main site (Church & Bloor St), Harbour Lights site and Specialized Care Centre located at (Victoria Park & McNicoll) and Remote Care Monitoring site (2 Overlea Blvd.). Position Summary:

The Director of Integrated Transitional Care at the Salvation Army, Toronto Grace Health Centre, is currently seeking a Program Assistant for the Remote Care Monitoring (RCM) program. The RCM program provides safety devices and monitoring services to older adults across Ontario, supporting clients to live safely and independently in their homes.

The Program Assistant provides operational and administrative support across the RCM Program, serving clients across Ontario and working under established procedures and the direction of designated program staff. The incumbent assists with referral intake, appointment scheduling, equipment coordination, client communication, documentation, record maintenance, and general program administration, providing flexible support across program functions based on operational requirements and client needs.

Hybrid Work Arrangement: This position follows a hybrid work model, with three (3) days per week onsite at the Overlea location and two (2) days per week working remotely. The hybrid work arrangement is subject to change based on organizational and operational needs.

Responsibilities:

Provide day-to-day assistance to assigned program areas based on operational needs and direction from program leadership
Assist Intake staff with referral processing, documentation, data entry, record maintenance, and routine client communication
Coordinate equipment installation appointments, communicate scheduling information to clients and caregivers, and provide basic equipment information using approved program resources.
Provide administrative support including responding to routine inquiries, maintaining records, preparing correspondence, and coordinating program documentation
Follow established procedures and processes, referring questions and non-routine matters to the appropriate program staff
Maintain accurate client and program records in electronic and paper-based systems
Enter, update, retrieve, and maintain information in databases, scheduling platforms, and other program systems.
Assist with preparation of routine reports and tracking documentation
Communicate professionally with clients, caregivers, healthcare providers, community partners, and internal staff regarding routine program matters
Other assigned duties, as required

Qualifications and Education:

Diploma or in Business Administration, Health Administration, Office Administration, Social Services, or a related field, or an equivalent combination of education and experience.
Minimum of one (1) year of experience in administration support, customer service, healthcare, community services, or a related field
Experience working in a healthcare, community, or social services environment is considered an asset
Strong organizational, time management, and prioritization skills, with the ability to manage multiple tasks and adapt to changing priorities
Demonstrated proficiency in Microsoft Office applications, including Outlook, Work, Excel, and Teams
Strong written and verbal communication skills, with the ability to communicate professionally and collaborate effectively with team members, clients/caregivers, community partners, and other stakeholders
Demonstrated ability to work independently and as part of a multidisciplinary team
Fluency in English (written and verbal) is required
