Summary:
The Nurse Case Manager is a member of an interdependent, multidisciplinary team serving clients with severe and persistent mental illness; provides client-centered assessments, treatment, rehabilitation and support services based on Reconnect program guidelines; is responsible for specialized functions including conducting mental and physical health assessments; health promotion and providing medication management and administration in conjunction with the Team Psychiatrist.
Responsibilities include, but are not limited to, the following:
- initiates service to clients according to established guidelines;
- treats clients by using correct nursing techniques that are in compliance with all legislated requirements, the practice and ethical standards of the College of Nurses of Ontario and specific Reconnect procedures and policies;
- participates in the on-going development of Reconnect services in terms of the medication room, supplies, medical records, resource files and policy and procedure issues;
- assists in the development and maintenance of effective linkages with hospitals and community service providers;
- assesses and evaluates client needs, and develops and revises client care plans accordingly;
- identifies and communicates significant abnormalities in the client data to the appropriate team members;
- ensures implementation and evaluation of case plans based on a team approach;
- identifies and reports irregularities or mistakes in client care to the appropriate team members, completes required documentation, and ensures appropriate follow-up;
- monitors the accuracy and timely completion of laboratory work;
- serves as a client advocate to ensure that quality care is provided to clients;
- works in the community as the primary case manager for up to ten [10] clients;
- provides program teams, clients, families and others with specialized consultation / education related to nursing issues;
- develops, implements and documents treatment plans in conjunction with the client and other program team members;
- co-ordinates and monitors the activities of the clients’ individualized treatment plans;
- maintains client charts and other related client and workload measurement documents according to established policies and procedures;
- develops clinically sound case plans using the Ontario Common Assessment of Need (OCAN) that are specific to the needs of clients and the objectives of the program in question;
- ensures implementation and evaluation of the care plans;
- attends and participates in staff meetings;
- provides counselling to assist clients in accomplishing their goals and managing their symptoms and day-to-day stressors;
- accesses, co-ordinates and advocates for services and resources to meet client needs such as housing, financial, vocational, educational, social, recreational, medical, legal, ADL, etc., per the treatment plan;
- consults with other service providers to promote continuity of care for clients;
- provides crisis intervention support to clients as required;
- provides support and education to clients’ families and social support networks;
- dispenses, administers and monitors medications, and keeps required records;
- provides clients with education and support regarding healthy lifestyles;
- assists clients with their ADL’s such as budgeting, shopping, meal planning and preparation, housekeeping, use of transportation, personal grooming, laundry, keeping appointments, etc.;
- participates in staff educational opportunities;
- participates in research and evaluation activities;
- participates in providing education to community service providers and other stakeholders regarding Reconnect programs, mental health and nursing-related issues;
- participates in committees and/or boards as required;
- may supervise students;
- adheres to all confidentiality laws and regulations, specifically the Personal Health Information Protection Act (PHIPA), as well as Reconnect’s confidentiality guidelines;
- follows all health and safety guidelines established by Reconnect’s Joint Health & Safety Committee;
- facilitates clients’ discharge when appropriate;
- fulfils other duties as assigned;
- delivers all services in a professional manner and according to established guidelines and associated benchmarks;
- follows all health and safety guidelines established by Reconnect’s Joint Health & Safety Committee.

Technical and Behavioral Competencies Required:
- graduate of an approved School of Nursing, providing eligibility for a current Certificate of Competence as a Registered Nurse from the College of Nurses of Ontario,
- minimum 3 years’ experience in psychiatric nursing; community psychiatric nursing preferred,
- strong knowledge of the nursing process and its application, including the assessment, planning, implementation, and evaluation of nursing care,
- a well-defined sense of diplomacy, including solid negotiation, conflict resolution, and people management skills,
- computer literacy, including effective working skills in MS Word, Excel and e-mail and able and willing to learn new software applications,
- excellent teamwork and team building skills,
- high degree of resourcefulness, flexibility, and adaptability,
- able to effectively communicate both verbally and in writing,
- very strong analytical skills,
- experience with substance abuse, dual disorder, dual diagnosis or legal issues would be an asset,
- able to work independently and in a team setting,
- fluency a second language would be an asset,
- knowledge of and sensitivity to gender, race, sexual orientation, age and multicultural issues,
- regular access to a dependable vehicle and a valid Ontario driver's license in good standing is required,

Workplace Requirements:
- May be exposed to infectious waste, diseases, conditions, etc., including exposure to the AIDS and hepatitis B viruses.
- Interacts with clients, family members, staff, visitors and other care providers.
- Generally employees work a 37.5 hour workweek.
- Work hours may vary depending on the needs of the client.
- Must be willing and able to work flexible hours.
- Attendance at special events from time to time will be required
