Care Team Representative

What we are looking for

Monitoring all bookings created by our members online, and by our call centre, Future Today.
These must be monitored at all times to ensure that patients are being booked with the appropriate doctors and that they have complete EMR charts prior to their appointment.
Patients must be called prior to their appointments and/or rescheduled and notified as required.
Following up with all non-cancelled appointments from the prior day. This includes sending texts to all prior-day patients, logging their responses and rebooking them for follow-up appointments as requested.
Following up with all emails from the call centre regarding patient inquiries. This includes appointment and prescription issues, general questions, etc. These must be monitored at all times to ensure timely responses.
Assisting the on-duty doctors with whatever they require.
Continually monitoring appointment volume and provinces to ensure we have enough doctors available to cover the bookings. Contacting and adding additional doctors as needed.
Managing all received documents. This includes receiving them, cataloguing them, adding appropriate notes, and uploading them to the proper patient charts.
Completing Clinical Intakes when necessary for our members seeking assistance with their mental health, as well as legal, nutritional, or financial counselling.
