Full job description

Position Summary:

As a member of the Community Wellness Services (CWS) division, this position provides professional support in improving access to services to meet basic needs, increase access to social supports, increase financial empowerment, and increase social inclusion for individuals, families, and communities.

The Wellness Facilitator will play a key role in providing longer term support to individuals and families. This includes: information and referrals, advocacy, support services, coordination of prevention and awareness of social programs and services, resource development, and establishing partnerships that enhance the quality of life for Chestermere and Area residents.

Key Accountabilities:

Provides professional level and program delivery expertise in the implementation, monitoring, and continuous improvement of:
Program and service planning, development, implementation, and coordination of support services: coordinates CWS programs, services, and events focusing on advancing equity and inclusion; completes intake and assessment requirements with clients to determine access and referrals to other programs and services; coordinates fundraising initiatives and partnerships related to poverty reduction priorities; increases community awareness of programs and services; receives and allocates community donations.
Networking, referrals and resource development throughout mandated service area: networks with community stakeholders and collaborates to increase knowledge of community resources; attends related information, committee, and coalition meetings; provides input into the development and implementation of CSS priorities for addressing social needs, and optimizing partnership opportunities.
Program reporting and evaluation: manages assigned contracts and execute reporting responsibilities, including financial and outcomes reports as assigned, regularly collects programming and service data, provides information on best practices around data collection tools; assesses, develops and/or coordinates new programs in response to community need; evaluates program impact by gathering input and feedback, utilizing evaluation tools, project budgets, analyzing and reporting statistical and evaluative data; provides support in exploring forward thinking cost and operating efficiencies, identification of financial risks and funding opportunities.
As required, participates in the enhancement and implementation of City policies and procedures within legislative, regulatory requirements and best practices, e.g., FCSS Act and Regulation.
Provides coaching, mentorship, and consultation to CWS staff and debriefing after client contacts.
Appropriate and responsible use of confidential information; maintains confidentiality and security of all information related to the City.
Ensures a safe and healthy work environment for staff, visitors, the community and stakeholders, e.g., participates in workplace safety initiatives, participates in formal and informal worksite safety and environment inspections and audits, ensures compliance with the City’s Health and Safety Management System, reports and assists in the analysis of near misses or incidents.
Participates as a member of the Chestermere Emergency Response team in preparing and enabling the community to respond in the event of a local or regional disaster.

Required education and experience:

Bachelor’s degree in social work or related discipline.
RSW Preferred
3 years of related and progressive experience.

Required certifications and/or documentation

Social Workers must be registered with Alberta College of Social Workers (ACSW).
Clear Criminal Record, Child Intervention and Vulnerable Sector Checks deemed satisfactory by the City.
Valid Class 5 Alberta Driver’s License and a Driver’s Abstract deemed satisfactory by the City.
Valid Standard First Aid CPR C and AED certificates, ASIST (Suicide Intervention) and Mental Health First Aid (MHFA).

Expected skills and attributes

Strong technical and advocacy skills when completing regional and government applications for subsidized services – seniors, childcare, disability benefits, low income, emergency funding, housing, etc.
Ability to respond in a calming and empathetic manner to clients experiencing difficult life circumstances.
Analytical and creative thinking skills; ability to solve problems, make decisions, negotiate and deal effectively with people in difficult situations.
Knowledge of governance issues e.g., policy development and interpretation.
Strong verbal and written communication skills; ability to maintain communication to anticipate and prevent potential problems, develop and maintain positive and effective working relationships with staff, community service providers, the public and other stakeholders; for completion of complex grant applications and agency reporting.
Strong work ethic and highly motivated; ability to work in a fast-paced environment and respond to action requests and competing deadlines in a timely matter.
Proficiency with MS Office (Word, Excel, PowerPoint, and Outlook).

Working Conditions:

Normal office conditions; long periods of sitting, reading and concentration.
Regularly working within the community and interacting with the public.
Regular travel throughout the region, sometimes in inclement weather.
Ability to lift and move items up to 40 lbs.
Regularly working after hours or weekends to attend community events, facilitate programs, and attend meetings.
Stress associated with risk management and the responsibility for the provision of professional support affecting the well-being of citizens.

Location: City Hall

Compensation: Pay Grade will be determined by position placement.

Pay: $31.09/hr (you'll find the detailed information in Appendix A of the collective agreement)
