Position Summary: Working in collaboration with Outreach Workers, the Link Worker will support older adults to live independently in the community and improve their overall health outcomes. As part of the Outreach Team, the Link Worker will provide the connection for older adults to a range of social, community and partner support to enable them to improve their health and wellbeing and to help them age safely in place as they choose. This could include assistance in accessing food resources, transportation, physical activities, volunteer opportunities or assistance in filling out forms. The Link Worker will provide direct support by providing information or attending appointments with the client. The goal of this position is to assist older adults to age safely in community.

Responsibilities

Working collaboratively with the Outreach Workers and Link Worker Lead to support older adults to access and engage with supports in the community.
Use social prescribing to empower people to take control of their health and wellbeing.
Spending time with older adults 1 on 1 to connect them to community groups and services for practical and emotional support and managing a caseload.
Provide ongoing support for an allocated but short-term timeframe to promote engagement with identified services and achievement of goals.
This role is a non-clinical role, however the Link Worker will receive support from both the Outreach Team and other agency staff.
Provide support for older adult needs in the community or via home visits, working as part of a team, supporting a caseload of older adults for whom social prescribing might offer improved outcomes.
Attend agency team meetings, Link Worker Community of Practice and other events as required.

Skills and Abilities

A commitment and ability to provide highly efficient, flexible support and encourage health and well-being services to older adults
A commitment and ability to work within a team environment
The ability to develop relationships with a wide variety of stakeholders
Excellent communication skills (both verbal and written) and experience in communicating to different audiences particularly within community settings
Ability to process and record information accurately and in a clear and concise manner
Experience working with people from a variety of backgrounds and abilities
Comfortable in difficult situations and working with vulnerable populations
The ability to analyze and creatively solve difficult problems, and to take initiative and make informed decisions
Ability to work independently and to manage time and caseload proficiently
Ability to work well with others (team members, older adults, community groups and partner organizations)
The flexibility to adapt to changing workload demands and new challenges
Undertake any tasks consistent within the scope of the role, ensuring that work is delivered in a timely and effective manner. Ensure compliance with all relevant policies and procedures in relation to health and safety, work alone legislation, data protection and confidentiality.

The successful candidate will have the following qualifications:

An undergraduate degree in a social science discipline, or related field of study is preferred
3-5 years of experience in a role dedicated to human services, building relationships with diverse and vulnerable populations
Excellent time management and problem solving skills with exceptional attention to detail.
Excellent technical and computer skills and ability to learn and utilize new software; experience with the Microsoft Office suite (i.e., Word, Excel, Outlook), virtual communication tools (i.e., Zoom, Teams) and case management software.
Satisfactory Criminal Record Check including Vulnerable Sector Check
Licensed and registered vehicle with $2 million auto liability insurance
Previous experience working within a social profit will be an advantage; experience working within a social profit dedicated to seniors is desired

Competitive salary and will be offered based on education and experience. Successful candidate will be required to provide 5-year Driver’s Abstract and access to a vehicle for use in their duties.
