The main purpose of this position

The System Navigator plays a critical role within the Interdisciplinary Primary Care Team, supporting individuals to access, navigate, and remain connected to primary care and related health and social services.

The System Navigator is the primary client-facing role responsible for supporting individuals through their care journey across the health and social service system.

This role acts as a key point of connection between clients, the primary care team, and system partners, helping reduce barriers to care and supporting continuity of services.

The System Navigator works closely with interdisciplinary providers to support client attachment, coordinate referrals, and ensure follow-up across the health and social service system.

This role operates within a trauma-informed, harm reduction, and client-directed model of care.

Responsibilities

System Navigation & Client Support

Serve as a primary point of contact for clients requiring support navigating health and social services.
Conduct intake and initial screening to understand client needs, priorities, and barriers to care.
Support clients in accessing primary care, including attachment to providers and ongoing engagement.
Assist clients in navigating referrals to specialists, diagnostics, mental health, addictions, housing, and community supports.
Provide follow-up support to ensure successful connection to services and continuity of care.
Support clients in preparing for appointments and understanding care plans.
Build rapport with clients using trauma-informed, harm reduction, and client-centered approaches.

Care Coordination & Interdisciplinary Collaboration

Work closely with the Medical Office Assistant to ensure coordinated scheduling, referral tracking, and continuity of client care.
Work closely with physicians, nurse practitioners, nurses, social workers, health promoters, and peer support staff to coordinate care.
Participate in case reviews, care coordination meetings, and team discussions.
Support the flow of information between clients, providers, and system partners.
Identify gaps and barriers in service access and escalate within the team as needed.
Contribute to integrated care planning across primary care, housing, and community services.

System & Community Partnership Support

Maintain up-to-date knowledge of community resources, referral pathways, and eligibility criteria.
Build and maintain relationships with community partners, including hospitals, shelters, and social service agencies.
Support coordination of services across organizations to improve client access and outcomes.
Facilitate communication between Support House and external partners where appropriate.

Administrative & Documentation Support

Document client interactions, referrals, and service coordination activities in the Electronic Medical Record (EMR) system.
Support accurate and timely data entry to ensure appropriate reporting and continuity of care.
Track referrals, follow-ups, and outcomes to support team workflows.
Assist in managing referral processes and coordinating scheduling with providers where needed.

Quality Improvement & Program Support

Support data collection and tracking of key service metrics related to access, attachment, and navigation.
Participate in quality improvement initiatives aligned with IPCT and organizational priorities.
Identify trends, service gaps, or operational challenges and contribute to solutions.
Support program development and refinement based on client and system needs.

Professional Development Work

Practice in alignment with trauma-informed, harm reduction, anti-oppressive, and client-directed care principles.
Engage in ongoing learning related to system navigation, primary care, and population health.
Participate in supervision, performance reviews, and team debriefings.
Engage in Communities of Practice and system-level learning opportunities.

Personal Development Work

Engage in reflective practice and maintain personal wellness strategies.
Communicate support needs and participate in wellness-promoting activities.
Maintain appropriate boundaries and manage dual relationships in alignment with organizational policies.
Utilize internal and external supports (e.g., EFAP) as needed.

Knowledge and skills necessary to be successful in this role

Post-secondary education in health, social services, community development, or a related field, or equivalent experience.
3–5 years of experience in system navigation, case coordination, or community health settings.
Strong knowledge of health and social service systems, including primary care, mental health, addictions, and housing sectors.
Experience working with individuals experiencing complex barriers, including homelessness and substance use.
Demonstrated understanding of trauma-informed care, harm reduction, and health equity frameworks.
Strong interpersonal, engagement, and communication skills.
Ability to build trusting relationships with clients and partners.
Excellent organizational and coordination skills.
Experience working within interdisciplinary teams.
Familiarity with TELUS PS Suite electronic medical record system
Knowledge of PHIPA and confidentiality requirements.
Ability to manage competing priorities in a dynamic environment.
Strong problem-solving and critical thinking skills.
Valid driver’s license and access to a vehicle required.
First Aid, CPR, and Naloxone training is an asset.

Working conditions

Full-time position (37.5 hours per week), with flexibility required based on program needs.
Primary work location is the Burlington clinic, with some community-based work as required.
Occasional evening or weekend work may be required.
Work is conducted in both clinical and community settings.
Support House prioritizes flexibility, collaboration, and work-life balance.

Pay: From $58,000.00 per year
