Full job description

Under the direction of the Director of Care, the Mental Health and Dementia Coordinator is responsible for inspiring, promoting, and overseeing the development and sustainability of a mental health and dementia program, ensuring emotional, and spiritual wellness to meet residents' needs within the framework of our compassionate, person-centred model of care.

Duties and Responsibilities

1) Coordinates and leads major programs; dementia and mental health, for SJHC long-term care homes through ongoing research, planning, development, organization, and evaluation.

· Coordinates systems and processes to support the development and sustainability of the mental health and dementia programs within SJHC LTC sites.

· Supports the mental health and dementia program through complex decision -making.

· Ensures the establishment and monitoring of best practices and quality standards.

· Develops, implements and revises policies and procedures.

· Develops and maintains collaborative relationships with community partners, staff, students, and volunteers to implement and maintain an effective program.

· Participates in identifying clinical or operational performance improvement and risk mitigation opportunities and assists in performance improvement activities.

· Designs and directs mental health education, awareness capacity and skill building.

· Participates in strategic planning and prepares written plans outlining the short- and long-range goals for the mental health and dementia program.

2)Provides functional, administrative, and operational support to the Director(s) of Care regarding all dementia and mental health programming within the organization for the LTC sites.

· Acts as an expert resource for managers, physicians, and other healthcare team members.

· Provides leadership and enforces organizational expectations to ensure all clinical staff adhere to facility policies, standards of care, and professional conduct.

· Supervises the mental health and dementia team.

· Lead with performance management issues, including attendance support, conflict resolution, performance audits, skills evaluation, performance appraisals, coaching and progressive discipline.

· Provide input into hiring, termination and performance evaluations.

· Mentors and coaches’ staff to ensure high levels of professionalism and adherence to care standard.

· Ensure that the team receives the appropriate development and education.

· Coordinates care and services across continuums.

· Travels from site to site to support, monitor, and develop the mental health and dementia program.

· Ensures compliance with the Fixing Long Term Care Homes Act, 2021, Regulations, and all other required legislation.

· Assists with the reporting processes and completes required documentation for mandatory reports, including but not limited to High-Intensity Needs

Qualifications

Bachelor’s degree in a health-related field.

Minimum of 1-2 years experience working with individuals and families in the mental health and dementia system; experience working in a Long-Term Care environment considered an asset

Knowledge of Mental Health legislation, Occupational Act: Health & Safety, Human Rights Code, Fixing Long Term Care Act, Privacy Legislation, and Understanding of Mental Health Policy considered an asset

Demonstrated effective critical thinking, analytical decision making and problem-resolution skills

Demonstrated expertise with professional practice and quality and ability to teach, coach and mentor individuals and teams

Excellent interpersonal and communication skills, planning and time management skills

Education/ Certification in Trauma-informed care, Mental Health First Aid, SafeTalk, Applied Suicide Intervention Skills Training, Mental Health Works, Nonviolent Crisis Intervention, Gentle Persuasive Approaches, Geriatric Certificate Program, Positive Approaches to Care, Dementiability, P.I.E.C.E.S, Montessori Dementia Certificate, or similar programs is considered an asset

Bilingualism in Official Languages is an asset

Basic Knowledge of computer Microsoft Office use such as Word, Excel, PowerPoint
