Full job description

Overview

The Pas Mobilization Steering Committee supports youth and adults through a consent based, culturally inclusive and client-centered wraparound model. We are looking for a dynamic individual with diverse skills to fill the position of Program Coordinator.

Qualifications

Post-secondary education in Social Work, Social Sciences, Applied Counselling, or a related field.
An equivalent combination of education and relevant experience may be considered.
Experience working with individuals with complex needs.
Familiarity with multi-agency and collaborative service delivery models.
Ability to work effectively with diverse individuals, families, and communities.
Strong interpersonal, communication, and conflict-resolution skills.
Experience with case management, administrative responsibilities, and report writing.
Ability to work independently while maintaining strong relationships with partner agencies.
Proficiency in Microsoft Office applications and data management systems.
Valid Class 5 Driver's License and access to a reliable vehicle.
Satisfactory Criminal Record Check and Child Abuse Registry Check.
Successful completion of an Enhanced Law Enforcement Security Screening.

Duties

Provide client-centered case management services to youth and adults with complex needs.
Conduct risk assessments and identify barriers impacting client well-being.
Develop and coordinate individualized intervention plans with clients and partner agencies.
Facilitate referrals and connections to community supports and services.
Participate in collaborative situation tables and inter-agency meetings.
Maintain accurate client records, case notes, and statistical information.
Prepare reports and documentation as required by the Steering Committee and funding agencies.
Support culturally safe and inclusive service delivery practices.
Promote positive relationships with community partners, service providers, and stakeholders. Occasional travel may be required.
Participate in program development, community outreach activities, and public awareness initiatives.
Perform other related duties as assigned.

Start Date: Contingent upon successful completion of Enhanced Law Enforcement Security Screening.
